Lobbyist Registration and Disclosure Report

The Lobbyist Registration Form and Disclosure Report can be completed and printed for filing with the City Clerk. Closing the form or choosing the "Clear Form" button will erase all information entered on the form.

We recommend you print two (2) copies.

• One for your records

The forms must be printed and signed and then mailed or delivered to the City Clerk's Office.
